  • Service List

  • List of locations you wish to subpoena

  • Wording for the subpoenas (“Attachment 3”) – to help you save time, we also have standard wording for subpoenas requesting medical/billing/radiological/pharmacy records, employment, DMV, insurance/claim files, Worker’s Compensation, and banking/credit cards.  Please let us know if you wish to use our standard wording for your subpoenas.

To place an authorization request by e-mail, simply e-mail us a copy of the authorization and be sure to include the location’s contact information and description of records to be retrieved.  For photocopying orders, please just e-mail over the location information and description of the records to be copied.

Due to company policy, we only work directly with law firms and insurance companies. Unfortunately, in pro per orders are not accepted at the current time.
